T.E.A.M.’s Crisis Residential Treatment (CRT) program provides a structured, supportive environment for individuals experiencing acute mental health challenges who need intensive stabilization.

Our licensed and DHCS-designated facilities deliver care consistent with California state standards, ensuring each member receives the level of clinical support they need in a non-institutional setting.

Reentry Support & Housing Navigation

Specialized support for justice-impacted individuals transitioning from incarceration or supervision. We coordinate safe, stable housing placements — including relocation outside high-risk environments when necessary.

Youth

Youth Housing & Crisis Intervention

Direct engagement with high-risk and disconnected youth experiencing housing instability. Immediate crisis support combined with long-term stabilization strategies and ongoing case management.

Trauma-Informed Case Management

Individualized, holistic service planning rooted in trauma-informed care principles. Personalized goal-setting addressing housing, education, employment, and mental health needs.

Behavioral Health

Mental Health & Therapeutic Linkage

Future Programs

T.E.A.M. is expanding its continuum of care with new programs designed to serve youth, transitional-age adults, justice-impacted individuals, and older adults.

Youth & Adult Mentoring Program

A preventative, community-based initiative open to youth ages 12–26. Programming includes etiquette and social skills, educational support, financial literacy, health education, parenting support, and Know Your Rights training.

Ages 12–26

Transitional Age Youth (TAY) Housing

Serving youth experiencing homelessness, involvement with the criminal justice system, child welfare, or psychiatric hospitalization. Housing options include emergency shelters, host homes, rapid rehousing, and rental assistance with ongoing mental health support.

TAY 16–25

Reentry Housing & Program

Structured residential and support programming for adults transitioning from incarceration. Focused on stable housing, employment readiness, substance use recovery, and long-term community integration.

Justice-Impacted

Residential Care Facility for the Elderly (RCFE)

A compassionate residential care program for older adults requiring non-medical supportive services. Designed to provide dignity, safety, and quality of life within a community-based setting.
